If you completed any course or workshop through the Maiberger Institute, a Certificate of Completion would have been provided to you once you met all the requirements for the course or workshop you attended.
To obtain another copy of your Certificate of Completion, please contact [email protected] and include the following in your email correspondence:

Fist and Last Name must identically match the name in our records, and the name listed on your state mental health license. Additional information and steps are necessary if your name has changed and/or does not match identically.
It is also imperative that you contact us from the email account you used when you attended the course in order to confirm your records. Additional information and steps will be required if you no longer have access to your original email account.
The replacement fee is $75:
- We do not verify records via phone/videochat.
- Only digital replacement copies will be provided (PDF)
- Physical printed replacement copies are not available.
- The digital replacement copy will be delivered via email.
